IURPA Password
Request Form

To request a User ID and Password, please fill out all of the required information on the form below. Please remember that unless you are a dues paying member, and input your "Member Number" you will not be able to "submit" your password request. If your request does not match current IURPA Membership data, your request will be denied.

As of April 1st, 2002, you MUST request a specific User ID and Password or you will not be able to "submit" the completed form. (The Web Master will no longer no longer make your choices for you).

If you have forgotten your Member Number, it is on the e-mail you are sent when your dues were received and deposited. If you cannot find that e-mail, you may email George Balsamo or Woody for it.

When they have been activated, your User ID and Password will be E-mailed back to you as confirmation. If they are incorrect and/or you want to change them, please fill out the form and submit again. Please allow time, as this is a manual process, and the Web Master is usually overloaded with requests at Dues time.... When you have completed the form, press the SUBMIT button at the bottom of the form.
